Prepare for Your Appointment

 

 To prepare for your appointment, please bring the following items with you:

• Complete new patient form and bring it to your appointment. 

• List all medications you are taking including dosage. This includes vitamins, herbal therapies, and all over-the-counter medications. (The medication list form included  with new patient form package) or you can bring the actual bottles if possible.

• Current insurance cards must be presented. You will be responsible for your copayment at the time of your visit. If you are not insured,  call the office to discuss your expected payment. (Insurance or other  financial information included with the new patient form package)

• Please arrive 15 minutes prior to your appointment time so we can have time to process your insurance and paperwork.

 

 

Medical Record:

If  you want us to release your medical record,  we would like you to fill the medical release authorization form. 

We may  collect payment prior to release the records. Please give us three business days for completion.

Cancel Your Appointment

If you are unable to make your appointment please call our office the day before by 3:00 p.m. to avoid a $25.00  no show fee. If you have any questions, please call the number  below.
